Direct Care Worker

Knowledge, Skills, and Abilities Required:

Job Duties:

Landscape Technician and Supervisor

Job Description
Landscape or maintain grounds of property using hand or power tools or equipment. Workers typically perform a variety of tasks, which may include any combination of the following: mowing, trimming, planting, watering, fertilizing, digging, raking, cleaning, hauling, repair structures, and maintaining outside of the buildings.

*Prerequisites:

  • Be able to function in a team environment.
  • Report to work on time.
  • Have your own vehicle to travel between locations.
  • Need a smartphone for time entry.
  • Driver's license.


Tasks Expected to perform:
Safely operate vehicles or powered equipment, such as mowers, tractors, twin-axle vehicles, snow blowers, chain-saws, electric clippers, sod cutters, or pruning saws.
  • Prune or trim trees, shrubs, or hedges, using shears, pruners, or chain saws.
  • Trim or pick flowers and clean flower beds.
  • Mow by hand and tractor.
  • Work in both Hot and cold temperatures.
  • Provide proper upkeep of sidewalks, driveways, parking lots, fountains, planters, or other grounds features.
  • Shovel snow from walks, driveways, or parking lots and spread salt in those areas.
  • Care for established lawns by mulching, aerating, weeding, grubbing, removing thatch, or trimming or edging around flower beds, walks, or walls.
  • Install equipment to protect or support trees.
  • Rake, mulch, and remove leaves.
  • Haul or spread topsoil or spread straw over seeded soil.
  • Plant seeds, bulbs, foliage, flowering plants, grass, ground covers, trees, or shrubs.
  • Mix and spray or spread fertilizers, herbicides, or insecticides onto grass, shrubs, or trees, using hand or automatic sprayers or spreaders.
  • Clean facilities or sites.
  • Clean job sites, gather and remove litter, dispose of trash or waste materials.
  • Use ladders to clean gutters and trim trees
  • Power wash surfaces (decks, sidewalks, siding)


Assist Maintenance Team where needed
  • Install rock gardens, ponds, decks, drainage systems, retaining walls, fences, planters, or playground equipment.
  • Decorate indoor or outdoor spaces.
  • Move Furniture

Occupation Therapist

Core Responsibilities

  • Perform skill acquisition assessments (ADLs and IADLs)
  • Develop skill acquisition goals and intervention plans (ADLs and IADLs)
  • Perform sensory assessments
  • Develop sensory therapy goals and intervention plans
  • Perform adaptive assessments and make professional recommendations
  • Attend program meetings, person centered planning meetings, and behavior management review committee as needed
  • Maintain a written record of assessments, interventions, and progress towards goals.
  • Consult with provider and treatment team regarding policies and procedures in clinical care
  • Consult with psychiatrist and other medical professionals as needed to coordinate medication with treatment plans


Additional Responsibilities
  • Attend group outings as available
  • Attend clinician and management meetings as required
  • Maintain and advocate for all Flatrock Manor residents' rights
  • Report daily activities to direct supervisor/executive director
  • Complete any additional task assigned by direct supervisor/Executive Director

Limited Licensed Psychologist

Core Responsibilities

  • Perform functional behavior assessments
  • Design and write positive behavior support plans
  • In-service treatment plans
  • Provide staff training in behavior strategies, positive interaction, structured routines, coping skills training and redirection strategies.
  • Consult with treatment team to problem solve and make programmatic changes
  • Attend weekly program meetings, person centered planning meetings and behavior management review committee as needed
  • Consult with provider and treatment team regarding policies and procedures in clinical care
  • Consult with psychiatrist and other medical professionals as needed to coordinate medication with treatment plans
  • Model and monitor program implementation
  • Provide supervision to floor leaders
  • Provide progress reviews on a quarterly basis at minimum
  • Maintain and advocate for all Flatrock Manor residents' rights
  • Complete any additional task assigned from the Executive Director

Journeyman Carpenters

Core Responsibilities

  • Install structures and fixtures according to specific instructions provided and ensure that all frameworks are properly installed
  • Measure, cut and shape wood, plastic and fiberglass according to provided instructions and construct frameworks such as wall studs and door-frames
  • Repair and maintain structures made from wood including windows, frames and flooring's
  • Inspect buildings and wooden structures to determine need for additions and repair and communicate information to the lead carpenter
  • Repair weather stripping and handle building interior and exterior maintenance work
  • Put up siding and drywall and ensure that scaffolding is properly erected and safely in place
  • Use a variety of hand and power tools to cut and shape wood and fiberglass in addition to properly placing them in desired areas
  • Build counter-tops and handle cabinet and furniture fabrication duties, along with installing metal doors and setting jambs
  • Perform interior renovations by performing repair and maintenance work on furniture and wooden framework

Salary: $18.00 to $30.00 /hour

Certified Medical Assistant

Core Responsibilities

  • Schedule all medical appointments including confirming and rescheduling if necessary
  • Communicate with physicians and medical offices to ensure appointments were completed
  • Coordinate services with Pharmacy including medication refills
  • Communicate with guardians/CMH as needed for medical issues
  • Check staff calendar to ensure all appointments are listed and no conflicts exist
  • Review medication administration records weekly
  • Perform medication cart audits weekly
  • Schedule monthly medication passer meetings within home staff to provide any updates, changes, etc.
  • Prepare necessary paperwork for all medical appointments and interactions
  • Print pharmacy orders monthly for emergency binder
  • Review discharge paperwork from hospital visits and send to appropriate physicians (i.e. PCP, psychiatrist etc.)
  • Ensure treatment is completed as necessary
  • Complete inventory of medications and medical supplies to reorder as necessary


Effective Teamwork & Communication
  • Maintain and advocate for all Flatrock Manor residents' rights
  • Complete any other task as assigned by the Healthcare Manager
